UVA Health Continuum Home Infusion, located at Colony Plaza on Fontaine Avenue, is celebrating 30 years of service to patients across Virginia and North Carolina.

Continuum provides patients with access to infusion therapy in the comfort of their own home. By eliminating the need to travel to a hospital or outpatient infusion center, patients can maintain their work, school, and family routines with less disruption. UVA Health Continuum Home Infusion expanded in 2025 with the launch of its Specialty Home Infusion Program, increasing access to therapies traditionally offered only in ambulatory infusion centers.

Our Multidisciplinary Care Team

While receiving care from Continuum, patients have a dedicated care coordinator who guides them from referral through treatment, including managing benefits, scheduling, and communication directly with their clinic. Our pharmacists, who specialize in biologics and chronic disease management, work closely with nurses who are skilled in intravenous and injectable medication administration and education.
