Parking Validation for Off-Site Vaccine Recipients | Anyone coming to the COVID Vaccine Clinic from an off-site location can ask their vaccinator for a parking validation ticket for the 11thStreet Parking Garage. Vaccine recipients working on-site at the Medical Center will not be offered parking validation tickets and should park in their designated parking area. One exception: On-site recipients who come to the clinic on their day off can request a parking validation ticket.

PPE UPDATE

N95 Mask Supply | While the worldwide supply chain for PPE has been constrained by the COVID-19 pandemic, UVA Health is working hard to protect our front-line teams by monitoring PPE supply inventory and use according to strict protocols. We currently have the PPE supply needed for our health care teams to properly care for our patients. The supply of the 1860S model of N95 is one that we are closely monitoring due to limited market supply. We are actively working to secure additional product, identifying appropriate alternatives for team members to use, and exploring every possible step to maintain and expand our supplies through new and existing vendor relationships:

PATIENT CARE

Ideal Pre-Operative COVID-19 Testing Time Frame | Although the testing standard is for patients to complete a PCR COVID-19 test within 7 calendar days before their scheduled surgery, ideally this test should be performed as close as possible to the scheduled surgery. The Asymptomatic COVID-19 Testing Center (ACTC) in the ERC offers access to the most ideal turnaround time of 2-4 days. The 7-day window was established to accommodate patients who need to obtain testing outside of UVA, which can have slower turn-around times for results.

Urgent Pre-Operative Testing Reminders:

UPDATE: Requests for Pre-Procedural Testing Expansion | Some proposals for inclusion in pre-procedural testing were received and are currently in review. Feedback will be shared beginning Monday, Jan. 11, 2021.

JOINT COMMISSION SURVEY READINESS

It’s Pull Cord Check Time | To help the Medical Center prepare for its triennial Joint Commission survey, which can happen anytime between now and early March 2021, please ensure that all pull cords in patient bathrooms are hanging freely and not touching the floor. Keep cords from being tied or wrapped around grab bars or toilet paper dispensers. Bottom line: Don’t let cords be stored in any way that would prevent a patient from quickly and easily pulling one to request assistance.

Challenges Met with Deep Compassion: A Night in the Special Pathogens Unit | Allison Kirkner, NP, recently worked a shift in the Special Pathogens Unit (SPU) and was thankful to have her UVA Health colleagues by her side through the difficult moments. She wrote the following message to thank the team for their extraordinary teamwork and patient care. We echo Allison’s appreciation and offer a heartfelt thank you to everyone caring for our COVID patients in the SPU and across the organization!

“[It] was the most challenging and most difficult night of my career. We literally ran from room to room and from emergency to emergency and unfortunately had to make some really difficult calls and have heartbreaking conversations with patients and families. I cannot express to you all how impressed and grateful I was for the nursing, respiratory therapy, and certified nursing assistant staff that were on that night . . . I saw so much compassion and caring that would be easy to have lost nine months into a pandemic. It is difficult to say anything that hasn’t been said about front-line staff at this point, but I was so proud to be working alongside everyone and would be grateful if any one of them were caring for me or my family.”

Pre-Operative Testing - Scheduling Guidance for Surgical Clinics | As Surgical Clinics assume the responsibility for scheduling their patients for pre-operative COVID-19 testing, the following tip sheets are available to guide team members through the new process for referring to the Asymptomatic COVID-19 Testing Center (ACTC):

Patient Considerations for Pre-Operative Testing | Patients referred to the Asymptomatic COVID-19 Testing Center (ACTC) at ERC should have no known symptoms or exposures, be able to self-collect a sample using a nasal swab, and be age 14 or over. Patients with COVID-19 symptoms, known exposure, ages 13 and under, or with other special considerations should be referred to an alternate testing site:

COVID VACCINE PROGRAM

Vaccine Program Expands to All Team Members | In an announcement yesterday, all team members were invited to schedule a COVID-19 vaccination appointment. The announcement marked the rapid expansion of the vaccination program, which began administering Pfizer vaccine to Group 1 (those at highest risk of work-related exposure to COVID-19) last week. All remaining team members have been prioritized into two groups:

OPERATIONS

COVID Staffing Communication Center Launching | As expected this holiday season, UVA Health is experiencing an increase in COVID-19 positive cases requiring an inpatient admission. In response to increasing COVID-19 volumes and the opening of an additional unit (3 South), we are launching the COVID-19 Staffing Communication Center (CSCC). The CSCC will collaborate with managers and leaders to expand the pool of team members able and available to support critical inpatient operations. At this time, we are requesting volunteers for paid assignments. See compensation details here. This situation is dynamic, and HRP 510 Modified Medical Center Operations provides guidance to support our patients and colleagues as needs evolve. FAQs and additional resources are posted on the Coronavirus Resources page beginning Dec. 24. Please talk with your manager, if you have any questions about opportunities to support inpatient staffing needs.

PATIENT CARE

Admission Testing Update – All Patients Will Be Tested for COVID-19 at Admission | All patients will now receive COVID-19 admission testing, unless they have had a positive infection within the last 90 days. Previously, patients who tested negative in the seven days prior to admission were excluded from this testing standard; however, the seven-day negative test exception has now been removed. All other exception inquiries should be routed to Infection Prevention and Control (PIC# 1883) for a determination. Admission Testing Guiding Principles and other workflow documentation have been updated and are available on the COVID-19 Resources page.

JOINT COMMISSION SURVEY READINESS

Report Critical Lab Values Promptly| Critical lab values and diagnostic findings have potential life threatening significance to the patient. These values should be reported promptly to the responsible LIP to ensure patient safety. For more specifics based on critical values or reporting pathways, refer to MCP 0125: Reporting of Critical Values and Findings and Critical Values and Timeframes for Reporting Guidelines.

COVID VACCINE PROGRAM

Roll-Out Expanding Rapidly | Look for an Announcement Email Tomorrow | The next phase of the COVID-19 vaccine roll-out is set to begin on Tuesday, Dec. 29, and all team members will be invited to participate. Before scheduling an appointment, team members must complete an online personal risk assessment and attest that they have reviewed the online Five-Step document. During the next phase of the roll-out, we will offer 3,000 vaccinations/week in the Education Resource Center while maintaining current vaccine clinic operations at Emily Couric Clinical Cancer Center.

Got Questions? | Learn as much as you can about the vaccine. Please access the information on the COVID Vaccination Program website; it will answer the majority of the questions you may have.

700+ Team Members Have Stepped Up to Receive Vaccine | The first work week of administering COVID-19 vaccine was busy indeed! In all, 737 team members were vaccinated and 1,911 appointments were booked through early January.

Volunteer Vaccinators Still Needed | RNs and pharmacists are needed to administer COVID-19 vaccines in a paid volunteer capacity. Volunteers will be offered the vaccine after completing three four-hour shifts (12 hours in total). Find more information on the COVID-Vax page

PPE UPDATE

New Donning and Doffing Tools Available | Visual aids to support team members with the proper steps for donning and doffing PPE are now available on the COVID-19 Resources page

OPERATIONS

Visitor Guidance Update | We are monitoring multiple data points to determine if and when additional changes to visitation restrictions will be needed. The key measure is the number of inpatients who are COVID-19 positive. Based on best practices, a review of visitation restrictions will be triggered if we have about 55 COVID-postive inpatients over a 3-5 day period.

Updated Pediatric Outpatient Visitation Policy | Starts Monday, Dec. 21 | To reduce the transmission of COVID-19 and protect, to the greatest extent possible, our patients, team members, and the community during the COVID-19 outbreak, routine patient visitation is restricted to designated visitors. The changes going into effect on Dec. 21 are intended to balance preventing the spread of COVID-19 with the needs of our patients and their loved ones and will align the visitation policies for pediatric inpatients and pediatric outpatient procedures.

COVID VACCINE PROJECT

Pfizer Vaccine Vials Hold More Doses Than Expected | According to a tweet from the Food & Drug Administration, “At this time, given the public health emergency, FDA is advising that it is acceptable to use every full dose obtainable (the sixth, or possibly even a seventh) from each vial, pending resolution of the issue.” We are putting those added doses to good use by vaccinating more team members sooner than originally planned.

Next Phase of Vaccination Rollout Begins Tuesday, Jan. 5, 2021 | Because our next vaccine allocations will be larger than anticipated, the COVID-19 vaccination program will be open in early January to all team members who work in patient-facing roles. This is a group of around 9,000 people, and their participation will mark a rapid expansion of our vaccination activities. More details will be coming soon.

COVID-19 Vaccinations Can Now Be Documented in Epic | In response to numerous inquiries, Epic has been updated to allow the entry of a patient’s COVID-19 vaccination record. Thank you for the quick work, Epic team!

Have You Been Vaccinated for COVID-19? | Share Your Story on the Vaccine Video Blog | Working in collaboration with the Virginia Department of Health, Sharecare is collecting stores from those who have received the COVID-19 vaccine. Kudos to Cross-Team Collaboration | Multi-Disciplinary Group Raced the Clock and Succeeded | When asked to stand up a testing site for pre-operative patients in under a week, who can deliver? The exceptional collaborative teams at UVA Health. These teams knew this would be a heavy lift with opportunities to continuously improve the process to meet the needs of providers, healthcare teams, and patients. The primary goal: achieve the best possible patient outcomes. Thanks to real-time problem solving and constructive feedback from across the organization, 130 patients have been tested to prepare for their surgeries in just two days!

COVID VACCINE PROJECT

UVA Health Begins COVID-19 Vaccinations for Front-Line Staff | UVA Health began vaccinations on Tuesday, Dec. 15, for employees at highest risk of acquiring COVID-19 through a work-related exposure, as a group that included doctors, nurses, environmental services workers, and pharmacists received their first of two Pfizer vaccine doses. When Will Other Team Members Be Vaccinated? | Based on current supply forecasts, we plan to begin offering vaccines to our 9,000 patient-facing team members during the month of January. All remaining team members will be eligible for vaccination shortly thereafter. Scheduling updates will be provided as planning progresses.

Dr. Ebony Hilton is vaccinated by Justin Vesser, manager of ambulatory pharmacy services. Hilton was the first person vaccinated at UVA Health.

Ready to Boost Your Area’s Hand Hygiene Performance? | As we all know, hand hygiene is the most important intervention to reduce the risk of illness transmission. It’s so important that each area’s hand hygiene performance is tracked on the UVA Health Dashboard. If you want to boost your area’s performance, make it a habit to do these two things:

  1. Stop for a Drop – Use hand sanitizer every time you enter and exit a patient room or bed space.
  2. Go Old-School – Use soap and water when there is a Green Handwashing sign outside the room or when your hands are visibly soiled. Otherwise use alcohol-based hand rub.

COVID VACCINE PROJECT

Vaccinations Set to Begin on Tuesday, Dec. 15 | UVA Health will receive its first shipment of Pfizer vaccine mid-day on Tuesday and will begin administering first dose shots to team members within hours. The first group of team members who are eligible to receive the vaccine are those whose work places them at highest risk of exposure to COVID-19, including physicians, nurses, medical techs, Environment of Care personnel, and others who work regularly in the Emergency Department and units that care for patients with COVID-19.

UVA COVID-19 Clinic Offering Monoclonal Antibody Infusion Therapy | Starts Today, Monday, Dec. 14 | UVA Health is offering a new ambulatory treatment option in the fight against COVID-19: Monoclonal Antibody Infusions (mAb). This investigational therapy is available under an FDA Emergency Use Authorization and may be considered for carefully selected patients who have mild to moderate symptoms and are at very high risk of disease progression or hospitalization. This therapy should not be considered the standard of care for any patient population

Because demand for mAb is expected to exceed supply, we have taken several steps to ensure equitable patient access based on clinical factors:

Suicide Prevention Assessment | Suicide is one of the leading causes of death in the United States. Assessing risk for suicide is one of the primary methods to keep high-risk patients safe while receiving care. Patients in the Emergency Department or on an inpatient unit must be assessed for suicide risk upon admission. Patients determined to be at risk for suicide must be placed on suicide precautions which include one-on-one observation by trained staff and removing potentially hazardous items from the environment.

Reminder: Patients Need Referrals to Access COVID-19 Clinic | Since the recent move to the first floor of the West Complex, the COVID-19 Clinic has been experiencing an increasing number of walk-ins sent to the clinic for testing. Please note: The COVID-19 Clinic is a referral-only testing site to protect the health and safety of patients and team members. The referral is an essential step that allows the clinic to schedule patients in a timely manner while preventing lines at the registration desk and crowded waiting rooms.

Prevent Patient Falls Through Continuous Risk Assessment | To keep patients safe, a Fall Risk Assessment must be completed by the inpatient RN within eight hours of admission and reassessed during every shift, with any major clinical change, or after a fall event. If a patient is determined to be at risk for falls, the RN must implement Fall Prevention Measures.

Yummy Protocol | Do You Know How to Handle Food Brought in for Patients? | Whenpatient food is brought onto inpatient units and needs to be stored/refrigerated, it should be dropped off directly at the nurses’ station. Before being placed in the Nourishment Room, a patient label and an expiration date two days from the current date should be affixed to the food or container. Food that has gone into a patient’s room cannot be stored or refrigerated in the Nourishment Room. For further information, please refer to the Nourishment Room signs on each inpatient unit.

GETTING AROUND

NEW: Patient Parking Reserved for COVID Clinic | Patients referred to the COVID Clinic, now located on the first floor of West Complex, should park in the West Complex surface lot, which is located in front of the main entrance and faces Jefferson Park Avenue. Several spaces have been reserved for COVID Clinic patients at all times and are clearly marked with signage. Team members are asked to not park in these spaces, including weekends and after hours.

Safety Alert: Check the Hazardous Medications List | Hazardous medications, such as chemotherapy and immunotherapy, require appropriate handling and disposal. The UVA Health Hazardous Medication list provides information on the personal protective equipment required based on the route of administration and whether the medication is listed as high or moderate risk.

Reminder: COVID-19 Clinic Appointments Are Referral Only | To protect patients and team members, testing appointments for the COVID clinic are referral only, and walk-ins are discouraged. This helps us to practice social distancing within our waiting room and avoids longer wait times for patients. Please place referral and test orders when referring patients to the COVID-19 clinic for testing:

VISITATION POLICY

Designated Visitor Can Enter Hospital Facilities Once a Day | As of last week, all adult inpatients can have only one designated visitor throughout their admission, and that person can enter and exit hospital facilities only once a day. If visitors leave the building, they are not allowed to re-enter until the following day. However, visiting the cafeteria or the Lee Street Pharmacy via internal walkways is not considered a departure.

GETTING AROUND

Parking on Lee Street Circle Is Prohibited | The circle in front of the Education Resource Center is not designated for use as a short-term parking area for patients picking up discharge prescriptions. All pharmacy customers should park in either the Lee Street or 11th Street Garages.

OPERATIONS

Reactivated Process: Access Team to Ask Patients About Travel | Begins Wednesday, Dec. 9 |  During registration, Access Team Members will be prompted to ask patients if they have traveled internationally or domestically in the last month and to document positive responses, noting locations and length of stay. Asking about recent international travel had been routine at the start of the pandemic, but later discontinued. The information gathered will help improve our understanding of travel-related COVID-19 infection, and will not fire a BPA at this time.

Lights of Love | On Saturday, Dec. 5, the University Hospital lobby holiday tree was illuminated for the Hospital Auxiliary’s 37th annual Lights of Love ceremony. This year’s virtual event featured Rhonda Meyer, President, University Hospital Auxiliary; Adam Slate, Intern Chaplain; andWendy Horton, Medical Center Chief Executive Officer. A special 3-year-old patient and guest of honor helped Santa light the tree.

Authorized ID Card or Key Needed to Access Academic Buildings | Saturday, Dec. 12-Sunday, Jan. 31, 2021 | Buildings around Grounds will be accessible only with an authorized identification card or key during the winter break. Current UVA ID cards have an embossed Rotunda and text printed beneath the magnetic stripe. Anyone who has an older card or one that does not work with contactless readers should get a replacement through the University ID Card Office. Contact your facilities coordinator for help with accessing interior doors with smart card readers and for arranging deliveries or other special needs.  

COVID VACCINE PROJECT

Missed the COVID-19 Vaccine Virtual Forum? Here’s the Link | On Wednesday, Dec. 2, Craig Kent, MD, Executive Vice President for Health Affairs; Wendy Horton, Medical Center CEO; and Costi Sifri, MD, Hospital Epidemiologist, hosted a virtual forum to discuss COVID-19 vaccines and the preparations for immunizing our workforce. Call for Volunteers to Administer COVID-19 Vaccines | RNs and pharmacists are needed to administer COVID-19 vaccines in a paid volunteer capacity. Volunteers will be offered the vaccine after completing three four-hour shifts (12 hours in total). Find more information on the Covid-Vax page

Help Prioritize COVID-19 Vaccination Groups | UVA Health will be offering the COVID-19 vaccine to team members in phases, as recommended by the Centers for Disease Control. In the first phase, the vaccine will be offered to team members who work in areas of highest risk, such as the Special Pathogens Unit and the Emergency Department. Planning for further prioritization will be based on responses to an online team member survey. Open to all team members, this voluntary and confidential survey closes on Dec. 18.      

JOINT COMMISSION SURVEY READINESS

Do You Handle Medical Waste and Sharps? | Handling regulated medical waste (RMW) requires adherence to state requirements. Liquid RMW should be disposed of in a toilet or hopper and not in sinks used for handwashing. All other RMW should be placed in a red bag container or sharps container for disposal.

CARING FOR OUR COMMUNITY

Community Testing Volunteers Needed | All Roles, All Dates | UVA Health has been providing about 300 COVID-19 tests per week to vulnerable community members in the Charlottesville area since the summer. Volunteers are needed to staff upcoming events as clinical testers, clinical assistants, greeters, traffic guides, and registration runners. COVID VACCINE PROJECT

Q&A Session for Managers and Leaders | Thursday, Dec. 3 | Noon-1 p.m. | Our first shipment of Pfizer’s COVID-19 vaccine could arrive in mid-December, if it receives Emergency Use Authorization (EUA) from the FDA, as expected. Because speedy administration of the vaccine will be of utmost importance, the first groups of team members will soon be notified about their eligibility to receive the vaccine, based on CDC and VDH guidance. The goal of this session is to clarify what is expected of managers and leaders during the vaccine roll-out and to answer questions they may have. A link to the meeting was included in an email sent today, and a recording of this session will be made available to those who are unable to attend.

Splish-Splash NOT! | On any given day, hundreds of clinical team members empty drains and canisters, participate in open suctioning procedures, and remove or open cleaning wipes. Mostly, these activities are safely completed. Some, however, result in a splash injury. What’s the best way to ensure safety and prevent injury? Take proper precautions, such as wearing a mask and eye protection in situations where spraying, splashing, or splatter to your face is possible. Refer to Safety First: PPE for Spray, Splash, and Splatter Exposures or the Standard Precautions portion of the Infection Control Manual for more details.

Free COVID-19 Testing Available | Saturday, Dec. 5, and Sunday, Dec. 6 | Noon-4 p.m. | Albemarle High School Parking Lot | The Department of Health and Human Services is providing 1,000 free, self-administered COVID-19 tests this weekend in partnership with UVA Health and the Blue Ridge Health District. While priority will be given to employees of Albemarle County Public Schools, Charlottesville City Schools, and UVA staff, the event is open to other members of the community who meet the following criteria:

Participants must arrive in a vehicle to receive their test, which will be administered in the back parking lot at Albemarle High School (2775 Hydraulic Road, Charlottesville).   2. Monitor and rotate supply inventories in primary and secondary storage locations to avoid supply excesses and to remove outdated supplies.

Change: Adult Inpatients Can Have Only 1 Designated Visitor | Effective Tuesday, Dec. 1 | In response to growing COVID infection rates within our community and the increase in COVID spread within families, UVA Health is taking added precautions to ensure the safety of patients and team members. Beginning on Tuesday, Dec. 1, all adult inpatients will be allowed to have only one designated visitor throughout their admission. The visitor will be allowed to visit only once a day. Visitation hours are from 8 a.m. – 9 p.m.

Updated PIN for Masking Personal Phone Numbers | Effective Tuesday, Dec. 1 | The PIN for phone number masking is now 146837. To ensure personal phone numbers are masked when making outgoing calls, team members should review the updated Masking Phone Numbers SOP which can be found in the Epic Learning Library. This process ensures that a UVA Health name and phone number displays when team members working remotely make outgoing calls.

Construction Near Lee Street Garage Will Impact Pedestrian Traffic | Monday, Nov. 30, to Friday, Jan. 15 | As part of an underground storage tank project, construction fencing has been installed on the north side of Lee Street between 11th Street and the entrance to the Lee Street garage. Pedestrians traveling toward the hospital will need to cross Lee Street at the intersection of 11th Street and Crispell Drive and use the sidewalk next to the hospital for access. Pedestrians traveling away from the hospital will be directed to cross at designated crosswalks and follow this same path in the opposite direction.

APPRECIATION

Kudos to the Pediatric Team for Addressing Food Insecurity in Our Community | At a pediatric surgery conference last year, Jeffrey Gander, MD, was inspired by the question, “What are you going to do in your community?” A year and a half later, his response is moving from vision to reality by providing regular deliveries of fresh produce to pediatric patients and their families who are experiencing food insecurity.

Gander and his partners – the Local Food Hub’s Fresh Farmacy program, Amy Wrentmore, MD, Associate Professor, Pediatrics; and Tegan Medico, Registered Dietitian/Nutritionist – have raised more than $8,000 so far, which will cover produce deliveries to 10-20 families for several months. To keep the program going, they have applied for a grant from the Robert Wood Johnson Foundation and are also accepting donations through the UVA Health Fresh Food Program fundraising page.

“All of us are in a profession to help people and care for people – a healing profession,” Gander says. “We owe it to our patients as part of our care for them to make sure that they have nutritious food. That may be even more powerful than some of their medications. Think about this fresh, healthy food as a way to treat their diseases.” See photos and read the full story on Connect.

COVID VACCINE PROJECT

Survey Responses Are Pouring In | So far, more than 950 team members have submitted their responses to the online survey that will help assess COVID-19 vaccination interest and prioritize vaccine distribution across UVA Health. Voluntary and confidential, the survey opened on Friday, Nov. 20, and will close on Friday, Dec. 18. Take survey.

Availability of COVID Vaccine Remains Uncertain | Many team members are asking when we will begin offering the COVID-19 vaccine to our high-risk work teams. The timing depends on when the FDA issues an Emergency Use Authorization to the first vaccine. This step must occur before vaccine distribution begins. According to the government agencies that are managing vaccine supplies, immunizations could start next month. At present, the earliest possible start date appears to be mid-December. The status of COVID-19 vaccines is rapidly evolving, and we will share updates as information becomes available.

JOINT COMMISSION SURVEY READINESS

A Quick Primer on Patient Rights | All patients — inpatient and outpatient — have a right to be informed about and involved in decisions related to their care and treatment. The Centers for Medicare and Medicaid Services have requirements, called Conditions of Participation, and one of them requires that hospitals inform patients of their rights. You can review a copy of the UVA Health patient rights document here.

Our steps to inform patients about their rights include: posting signs in the lobby and near entrances and elevators in all patient care facilities; providing a copy of the document online at uvahealth.com, in the patient education repository, and in the patient handbook (that is provided to each hospital inpatient); and, offering a copy during registration to all outpatients at least once a year.

Let’s Celebrate the Return of Hope | Peggy Plews-Ogan, MD, co-director of the Wisdom and Well-Being program, noted that she felt something stirring after Dr. Costi Sifri’s presentation on UVA’s plans for the COVID-19 vaccine, which could be available soon. In remarks made at last Thursday’s Medical Center Management Huddle, she explained that she was beginning to feel a sense of hope. Remember hope? It’s something not felt by many of us for a while. 

To honor and celebrate the return of hope, Dr. Plews-Ogan shared the following observation from The Gifts of Imperfection by Brené Brown, PhD (Hazelden Publishing 2010). Hope happens when:

  1. We can set realistic goals. (We know where we want to go.)
  2. We are able to figure out how to achieve those goals, including the ability to stay flexible and develop alternative routes. (We know how to get to that goal, and we are persistent and can tolerate disappointment and try again.)
  3. We believe in ourselves. (We can do this!)

Here’s to the return of hope! Like a bright, yellow flower growing through the cracks in concrete, may it brighten our spirits, empower our lives, and bring added purpose to the work we do every day!

Recruiting RNs and Pharmacists to Immunize Team Members |In preparation for the arrival of our first shipment of COVID-19 vaccine, RNs and Pharmacists are being recruited to serve as paid vaccinators in our soon-to-open COVID-19 Vaccination Clinic in the Emily Couric Clinical Cancer Center. Vaccinators are asked to volunteer for at least 12 hours (three 4-hour shifts) outside of their normal work hours. After working 12 hours, vaccinators will be eligible to receive the COVID vaccine. Check For Out-of-Date Equipment Stickers | Medical equipment used in patient care must be determined to be safe for use by Clinical Engineering. Once a piece of equipment has been determined to be safe for use, a sticker is placed on the item. Some pieces of equipment are required to be checked only once and some require annual checks. Equipment with annual check requirements should have a colored sticker in place. Those stickers read like a vehicle inspection sticker. If you find a piece of equipment without a sticker or an out of date sticker, remove the item from patient care, place a maintenance tag on the equipment, and contact Clinical Engineering at 434.924.2391.

What a Remarkable Job, Asymptomatic Inpatient Testing Team! | In its first four days, our new asymptomatic inpatient testing program ramped up in an amazing way: 471 inpatients were tested, and five were identified as COVID positive. The project marks the latest milestone in our ongoing and expanding efforts to keep teams and patients safe as the COVID-19 pandemic accelerates.

Transplant Administrator Rob Teaster, who led the launch, credits the project’s success to the collaboration of many groups, including the Emergency Department, Lab, Nursing, Nursing Administration, Supply Chain, Hospital Epidemiology, Infection Prevention and Control, Epic and Informatics. COVID-19 Admission Testing Tips | When placing COVID-19 testing orders for a patient being admitted (even from a post-op environment), LIPs should select asymptomatic admission testing to ensure a priority turnaround. Selecting “pre-procedural” could delay the test being run, because it is received with a lower priority. Also, please place the second patient label (identical to one on tube) on the outside of the third bag, making sure the label can be easily seen. This label indicates the reason for testing and priority to the lab team.

Free Communicator Masks Available For Interacting With Patients Who Are Deaf or Hearing Impaired | Team members who interact with patients who are deaf or hearing impaired are encouraged to wear a communicator face mask, which has clear plastic to show your mouth. Communicator masks —- as well as traditional face masks —- are available for free from Volunteer Services, which has so far distributed about 10,000 handmade masks to our team members.

Get Up to Speed on Manufacturer’s Instructions for Use | During their upcoming visit, Joint Commission surveyors may ask questions like: How long is the contact time for my cleaning solution? IMPORTANT ANNOUNCEMENT

COVID-19 Vaccine Will Be Available to Interested Team Members in High-Risk Roles | UVA Health will be receiving COVID-19 vaccine to immunize our high-risk team members. We expect one or more vaccines to be made available through an Emergency Use Authorization (EUA) issued by the FDA with distribution controlled by the Centers for Disease Control and managed within the state by the Virginia Department of Health.

At this time, we do not know which vaccine will be approved first, how many doses will be available, when the first shipment will arrive, and how frequently our supply will be replenished. Nonetheless, we are rapidly developing plans to offer the vaccine to interested team members when it becomes available and as advised by the CDC. We have ordered and received two sub-zero refrigerators for storing the vaccine developed by Pfizer, which is entering the final stage of approval. Look for updates next week.

PPE Protects! Safety Notice | What we learn about COVID-19 is evolving, but what we know about PPE and how it protects us, our patients, our families, and our community is clear. Proper PPE protects! We must be diligent and follow the Universal Masking, Eye Protection, Respirator Use Policy to limit the risk of exposure and ensure we stay healthy and available to provide care for the patients whose lives depend on us.

New COVID-19 Testing Protocols for Admissions | Effective 10 a.m., Monday, Nov. 16 | UVA Health will further limit the risk of exposure for patients, faculty, and team members by providing COVID-19 testing to all patients admitted to the Medical Center. The following resources have been developed to ensure you have the resources you need to make testing decisions and provide quality care in our changing environment.

Additionally, starting on Tuesday, Nov. 17, we will begin a testing sweep of currently admitted patients. Operational details will be shared with those impacted areas. Thank you for your ongoing dedication and flexibility as we navigate through the changes that allow us to deliver the safest, expert care for our patients. JOINT COMMISSION SURVEY READINESS

No-No Alert: Under Sink Storage | Storage of patient care items under sinks is prohibited due to risk for potential water leaks contaminating supplies. In outpatient areas, only closed cleaning products are permitted to be stored under sinks. Reminder: Planned Sanitary Sewer Outage This Weekend | University Hospital East Patient Tower | 11 p.m., Saturday, Nov. 14 - 3 a.m., Sunday, Nov. 15 | Facilities Management will shut down three main sanitary waste/drain risers in University Hospital East Patient Tower ONLY as part of the hospital renovation project. During this outage, please do not pour liquid in sinks or flush toilets. The Facilities team will post signs on doors of impacted patient rooms and common area bathrooms. Impacted toilet tanks will be covered with trash bags as a reminder not to flush during the outage. On 5 East, patient rooms will have cardboard placed over the sink and toilets as a reminder.

Unit managers have been provided an Incident Action Plan (IAP) with details about this outage. This IAP also identifies which rooms will be available for handwashing and toileting needs during the outage.

Care for Caregivers Program | Recently launched, the Care for Caregivers program provides an array of services for healthcare workers across Virginia who may need support for behavioral health needs including traumatic exposure, family strain, burnout, and more. Preventing Mistakes in Surgery | One of the key ways we prevent mistakes in surgery is by following Universal Protocol. The term Universal Protocol applies to any surgical or invasive procedure that requires informed consent, exposes the patient to a risk of harm more than routine care, and involves the puncture/incision of the skin (excluding venipuncture) or insertion of a foreign material into the body, in accordance with MCP 0250: Universal Protocol for Preoperative/Pre-procedure Verification.

Universal Protocol Compliance is as Easy as 1-2-3

  1. Verify that the correct surgery is being done on the correct patient and at the correct place on the patient’s body.
  2. Mark the correct place on the patient’s body where the surgery is to be done.
  3. Perform a “time out” before the surgery to make sure that a mistake is not being made.

Intravenous Fluids and Sterile Water for Inhalation | Being prepared for our upcoming Joint Commission survey requires the appropriate storage and use of intravenous fluids and sterile water for inhalation. Here are two key things you need to know:

  1. Keep bags in the overwrap until ready for use in the patient’s immediate care area. If you have questions about a specific product, please refer to the manufacturer’s instructions for use.
  2. Discard unwrapped bags that will not be used immediately. Unwrapped bags, even if not spiked, are considered a potential infection risk. Please discard any fluid or sterile water that is found without overwrap in work areas, or storage locations.

Launch: PreCheck-In Pilot | Begins Monday, Nov. 9 | Part of Epic MyChart Patient Access Enhancement Program | Sixteen Epic departments in 12 clinics will begin a pilot to assess the use of PreCheck-In in MyChart. The feature allows patients to update personal and demographic information, e-sign certain documents, and update medications, allergies, and health issues within a single workflow. Patients will receive a push notification three days prior to their scheduled appointment and can update their information until the time of their appointment. Moving: COVID Clinic | Saturday, Nov. 14 | The COVID Clinic and COVID Cohort Phlebotomy Lab are relocating to the first floor of the West Complex. Honor Walk for Departing Lung Transplant Patient | Exciting. Heart-warming. Joyous. Those are some of the words that describe the recent send-off that our first lung transplant patient with COVID complications received when he left the hospital. As Rob Teaster, the Transplant Administrator explained, this patient was a former tri-athlete who had been transferred here from another hospital suffering from the effects of COVID-19. To celebrate his recovery in a memorable way, the Transplant team organized a socially distanced, modified honor walk. 

Teaster applauded the bravery and skill of the Lung Transplant Team as they became one of the first in the U.S. to perform this life saving procedure for a patient with COVID-related ARDS/Fibrosis. He also expressed his appreciation to Bush Bell and his team for assisting with the celebration on such short notice and encouraged other teams caring for patients with COVID-19 to find inspiration in this patient’s successful recovery.

COVID-19 Access BPA Update Set for Tuesday, Nov. 3 | To align with CDC guidance, additional symptoms will cause the Access BPA to fire, potentially causing the BPA to display more frequently. RNs to Join LIPs on Callback Team for Positive COVID-19 Test Results | On Monday, Nov. 2, a process change will enable RNs as well as LIPs to handle callbacks to clinic patients with positive COVID-19 test results. In addition to reducing callback volume for LIPs, this change is expected to expedite communication of this critical test result. When RNs make callbacks, LIPs will provide support, serve as an escalation pathway, and ensure timely communication of test results. LIPs will retain ultimate responsibility for informing patients about positive COVID-19 test results.

RECENT EXPOSURE EVENT

This weekend, UVA Health experienced a COVID exposure within Heart and Vascular due to the treatment of a very complex asymptomatic patient. The circumstances related to this event were extremely unique and were not due to a lack of PPE supplies, failure to adhere to universal masking or PPE protocols, or related to any team member reporting to work sick. As part of its contact tracing efforts, Infection Prevention and Control (IP&C) is in the process of contacting any individual who may have been impacted.

We will be providing an update on this event in the coming days. This complex situation has also given us the opportunity to review our policies and procedures related to pre-procedural/pre-surgical testing. UVA Health leaders and providers are collaborating on the best protocol to ensure we are able to provide the safest care possible. Updates on related policies and protocols will be forthcoming.

GETTING AROUND

Magnet Replacement Project at Fontaine to Impact Parking | Saturday, Nov. 7, 6 a.m.-Sunday, Nov. 8, 6 p.m. | Crews will be using a crane to remove an old MRI and install a new MRI on the first floor of 415 Ray C. Hunt Drive. To allow for material to be delivered and staged in the work zone, the parking lot in front of the 415 building will be roped off on Saturday and reopen when work is complete and safety is restored in the area.

Emotional Support Services Available to Physicians, Nurses, and Other Healthcare Professionals | The Population Health team is offering short-term (1-6 sessions) using intervention protocols — such as Eye-Movement Desensitization and Reprocessing (EMDR) — that are specifically designed to address stress, negative thoughts, and negative feelings directly related to COVID-19. Mind Your Ls & Cs (Logs and Checklists) | Logs and checklists are an important way to ensure standard processes for maintaining equipment and supplies are routinely monitored for proper functionality. Examples of logs and checklists include emergency equipment checklists, eyewash station checklists, and medication refrigerator temperature logs. It is essential that logs and checklists are fully completed per the instructions provided. Deviations from expected observations, such as a temperature out of range, should include documentation of the corrective action taken and any follow up monitoring completed.

UVA Community Testing Provides Thousands of Free COVID-19 Tests in Charlottesville (UVA Today) | “We have learned that many people feel more comfortable getting tested in their own environment, at a church or a neighborhood they are familiar with, over coming to UVA to get tested,” said Mohan Nadkarni, MD, chief of UVA’s Division of General Medicine, Geriatrics, Palliative Care and Hospital Medicine and the medical lead for the community testing program. “We have had a great turnout so far, and it’s been great to be able to respond to those needs.” The community testing program began this summer, and more than 3,000 tests have been performed so far. On average, each site administers about 75 to 115 tests per outing, usually over about a two-hour period.

Q&A: Dr. William Petri on the Coming COVID-19 Surge (UVA Today) | William Petri, MD, the Wade Hampton Frost Professor of Medicine at the University of Virginia and vice chair for research in the Department of Medicine, is studying the effects of COVID-19 on the immune system and seeking new treatments and vaccines. Watch the Clock, Please! | As we prepare for the Medical Center’s upcoming triennial Joint Commission survey, team members are reminded to watch the clock when using a disinfectant to clean patient care equipment and surfaces. After application, each disinfectant requires a specific ‘contact time’ during which a surface must remain wet to effectively kill organisms. Contact times are noted on the disinfectant container. Cancer Center Loading Dock Closure | Wednesday, Oct. 21 | 5-8 a.m. | Crews will be closing the loading dock area behind the Emily Couric Clinical Cancer Center to install a crane and deliver materials to the roof. During that time, access via Jefferson Park Avenue will not be available to pedestrians and vehicles; Lee Street and W. Main Street sides of the building must be used instead. If work is completed early, normal activities will resume when safety is assured.

UVA Ranks 18th Among ‘Top 50 Teaching Hospitals for America’ (UVA Health Newsroom and Connect) | Washington Monthly Magazine has recognized UVA Medical Center as the 18th best teaching hospital in the country in its 2020 edition of the “Top 50 Teaching Hospitals for America.” The Medical Center received this distinction based on excellence in patient care, civic leadership, and community benefit and inclusivity. UVA was also ranked as the top hospital in Virginia.

Inside the Science of Testing Wastewater at UVA for Evidence of COVID-19 (UVA Today) | “What we’re looking for isn’t the virus itself, but specific RNA remnants of the virus,” said Amy Mathers, MD, an infectious disease physician at UVA Health and associate director of clinical microbiology for the UVA School of Medicine. “And it turns out that this method is very accurate for finding out if the people living in the building and using the wastewater system have COVID.”

UVA's Orthopedic Center Set to Open in 2022 (CBS 19) | UVA's orthopedic center is set to be complete in 2022, and Bobby Chhabra, MD, chair of orthopedic surgery at UVA, says it's been years in the making. "That facility will house all our clinics including the orthopedic hand center, foot and ankle center, orthopedic spine center, orthopedic joint replacement center, orthopedic trauma center and orthopedic sports medicine center. It will have physical therapy, occupational therapy, prosthetic orthodontics division, outpatient rooms for same-day joint replacement and sports surgery and will have advanced imaging such as MRI's and CT scans," Chhabra said.

Primary Care Doc Grateful for HIT Support | As Dr. Cate Varney of Zion Crossroads Primary Care explains it, delivering primary care and obesity management services to our patients during a pandemic is imperative. Priorities include making sure patients with obesity are not backsliding and determining if those with congestive heart failure need early intervention. 

While many patients have been using smart watches and similar devices to track and transmit their weight and other vital sign data to the clinic, that information was not automatically loading into the flow sheets and graphs used to monitor their status. Seeking a solution, Varney turned to Collin Murray and his colleagues on the Health Information Technology Epic team. 

“In less than one week of my request, the IT team developed a solution that makes it easier to track a patient's weight, along with other vitals, while delivering telemedicine services,” she reports. “I wanted to express my appreciation for Collin and his team in the Epic IT department. I’m so grateful and appreciative of all they do.”

It’s Pull Cord Check Time | To help the Medical Center prepare for its triennial Joint Commission survey, which can happen anytime between now and early March 2021, please ensure that all pull cords in patient bathrooms are hanging freely and not touching the floor. Keep cords from being tied or wrapped around grab bars or toilet paper dispensers. Bottom line: don’t let cords be stored in any way that would prevent a patient from quickly and easily pulling one to request assistance.

Inside UVA’s Evolving Strategy to Test for COVID-19 (UVA Today/Connect) | Every afternoon this fall semester, teams at the center of UVA’s coronavirus strategy gather on Zoom video conferences to plan the hours and days ahead. In typical understated fashion, Dr. Mitch Rosner, chair of the Department of Medicine and a constant presence across the various Zoom sessions, summed up the work at hand to his colleagues during a recent prevalence testing call. “We’re trying to balance a lot of things,” he said after a lengthy review of the day’s situation.

Reminder: Patients With Respiratory-Related Medical Conditions Can Use Alternative Masks | Effective Sept. 9, 2020, the Clinical Guidance for Patient Mask Use states that patients and visitors who are over the age of 2 and have a medical condition that prohibits mask use should be seen and receive care in clinic if the patient agrees to wear an acceptable alternative mask, or full face shield. Otherwise, telehealth options should be offered. (Note: Please review the guidance document for exceptions for urgent care.) Patient personal preference is not a legitimate reason to allow mask alternatives.

Honor Walk Draws Gratitude | “Last night, we took dinner and visited with a friend who recently lost his wife. We were talking about her last few days, her decision to be an organ donor, etc. She had multi strokes (before and after being admitted) and eventually passed away. UVA let her husband and children take part in her Honor Walk. The husband shared with me that the Honor Walk was such a blessing. I remember him saying that it was not clapping and cheering, but a moment of silence to reflect on and to honor her life and choice to be an organ donor. It brought the family closure and peace. I am so thankful that UVA took the time to do this. I can’t put into words how much it meant to them. I hope UVA continues to take the time to do this. The Mindful Leadership Program is a weekly series of eight webinars led by an eMindful expert teacher that will take place Mondays, 4-4:45 p.m., between Oct. 26-Dec. 14. The curriculum weaves in the leadership competencies for UVA's Cornerstone Program and UVA Health's ASPIRE values. Voter Registration Table | Oct. 7, 9, 12, and 13 | 10 a.m.-noon and 2-4 p.m. | To support team members in registering to vote, senior leadership has given the League of Women Voters the go-ahead to staff a passive voter registration desk in the pedestrian link in front of the Primary Care Center. There will be no proactive registration, and the process will remain non-partisan at all times.

UVA Launching Project to Determine Long-term Brain Effects of Blast Exposures in Military Personnel (UVA Health Newsroom and Connect) | The UVA School of Medicine researchers are partnering with the U.S. Navy and National Institutes of Health to develop a model predicting how regular exposure to artillery blasts affects the brains of military personnel.

UVA Testing Sound Waves to Address Erectile Dysfunction (UVA Health Newsroom and Connect) | UVA is exploring the potential of “low-intensity shockwave therapy” to address erectile dysfunction by promoting blood vessel growth and tissue repair.

HIV Drugs Could Prevent Diabetes, Study Suggests (UVA Newsroom and Connect) | A new study led by Jayakrishna Ambati, MD, of the School of Medicine suggests that a group of drugs used to treat HIV and hepatitis B could be repurposed to prevent type 2 diabetes.

Remember: Sticky Is Icky | We are in the window for the Medical Center’s triennial Joint Commission survey and are asking all team members to help us prepare for success. If you need to hang a temporary sign, avoid using common office tape or medical tape. Why? When removed, these tapes leave behind a residue that cannot be effectively cleaned and may pose an Infection Control risk. Acceptable alternatives are products like command strips or reusable adhesives.

PPE UPDATE

Clean Shaving Still Required | Our COVID-19 precautions are still in effect, and all team members required to wear an N95 mask must be clean shaven. In order to preserve PAPR supplies, switching to that equipment is not an option for those who want to avoid shaving. 

Thank you to all who normally grow facial hair, especially during the colder months. Your compliance with our COVID-19 PPE grooming requirements is truly appreciated. As always, our goal is to protect your personal safety and that of our patients, teams and community.

Help Needed: Nurses and Other Team Members Licensed to Provide Flu Shots | Paid Shifts for Volunteered Time | UVA Employee Health is seeking nurses and other team members who are licensed to provide flu shots to volunteer for paid shifts during which they will vaccinate UVA Health work groups. Due to COVID-19 precautions, this year’s flu plan focuses on vaccinating team members in their work areas rather than holding large flu clinics. Inpatient units and ambulatory clinics are being provided with To Go Kits so they can vaccinate their own teams.
