As we celebrate UVA Health Case Management Week from Oct. 13-19, it’s important to recognize the invaluable contributions of case managers and the case management team in the acute care hospital setting. This week serves not only as a celebration of their dedication, but also as an opportunity to reflect on the critical role they play in ensuring high-quality patient care.

Understanding Case Management

Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s health needs. In the acute care hospital environment, case managers serve as essential navigators, guiding patients through the complexities of healthcare while ensuring they receive the appropriate level of care. The UVA Health case management team is made up of nurse case managers, social work case managers, discharge planners, and the administrative specialist.
