UVA Health Continuum Home Health Earns U.S. News & World Report Highest Ranking
UVA Health Continuum Home Health is among the top 17% of home health agencies nationwide to earn the “High Performing” rating — the highest distinction awarded by U.S. News & World Report in its inaugural home health ratings.
U.S. News recently completed inaugural ratings and profiles for nearly 8,000 home health agencies. Based on a national survey of patients and family members, agencies must deliver high-quality care and excellent patient experience to earn the “High Performing" rating. They look at factors such as professionalism, communication, overall satisfaction, and care outcomes. Continuum received a 5 out of 5 patient experience score!
Continuum has served the community since 1995. It is accredited by the Joint Commission and is Medicare- and Medicaid-certified to provide services throughout Virginia. The team delivers in-home care to people of all ages with disabilities, chronic or terminal illnesses, and those facing a long recovery from acute illness.
Each patient receives a designated care manager who coordinates care with their physician and hospital discharge planners. It requires team members who enjoy people and solving complex care in unique and sometimes challenging home environments. The team includes nurses, pharmacists, therapists, social workers, nutritionists, aides, and other members of the Population Health Department.
As an academic home health agency, Continuum also seeks to educate, share knowledge, and help develop future home health clinicians.
In June 2024, Population Health, led by Novella Thompson, MBA, MA, ALM-C, FACHE, expanded to include teams from Continuum Home Health, Home Infusion in partnership with UVA Health Pharmacy, Transitions of Care, and Post-Acute Care operations — creating a unified strategic direction for managing care for our patients outside our immediate facilities.
